Who is the controller?
The controller is Smile Centrum s.r.o., Company ID (IČ): 03496902, registered in the Commercial Register maintained by the Municipal Court in Prague, Section C, Insert 102854, with its registered office at Trmická 835/4, 190 00 Prague 9, Czech Republic, which processes your personal data for the purposes set out below.

Purpose of personal data processing
Your personal data are processed for the following purposes:
online appointment booking (based on the appointment form),
provision of dental services (based on the registration form),
direct marketing (based on your consent).
How data are collected
When booking an appointment, we collect the following personal data:
first name and surname,
description of the requested treatment or problem,
telephone contact,
e-mail address,
date, time and place of the requested appointment.
Personal data for direct marketing
If you have given consent for direct marketing, your personal data are used to send commercial and non-commercial communications to your e-mail address.
We process in particular:
first name and surname,
e-mail address,
date of birth.
With whom we share your personal data
Your personal data are processed through our software system, accessible only to authorized employees of the controller.
Personal data used for direct marketing are not shared with third parties and are used exclusively by our company.
We have also concluded a joint controllership agreement for the purpose of improving service quality. Based on this agreement, your personal data may be shared with ProSmile Clinic s.r.o. and Family Dental Care s.r.o.
Transfer of personal data abroad
Your personal data are not transferred outside the Czech Republic or the European Union.
Retention period
Personal data are stored only for as long as necessary to fulfil the purpose of their processing. Data related to dental services are stored in accordance with applicable legal requirements. Data processed on the basis of consent are stored until consent is withdrawn.
Access to personal data and rectification
You have the right to request access to your personal data at any time. You also have the right to obtain confirmation as to whether your personal data are being processed and, if so, to obtain a copy of them.

In case of changes to your personal data, you may request their update or correction. We will rectify or complete your personal data without undue delay upon receipt of your request.
Erasure and restriction of processing
You have the right to request erasure or restriction of processing of your personal data at any time, either by e-mail or by post at the address above.
Personal data processed on the basis of consent for marketing purposes will be erased without undue delay upon receipt of the request or upon withdrawal of consent.
Data portability
If you wish your personal data to be transferred to another controller, you may request their transfer. The data will be provided to you without undue delay in a structured, commonly used and machine-readable format.
Objection to processing
You have the right to object to processing of your personal data for marketing purposes. In such case, your personal data will no longer be processed for these purposes.
Automated decision-making and profiling
Your personal data are not used for automated decision-making or profiling.
Withdrawal of consent
You may withdraw your consent at any time, in any form, by e-mail, post or in person.
Withdrawal of consent does not affect processing based on legal obligations or provision of healthcare services.
